[x] get all tests to run for PHP 7.3 in order to establish baseline of what tests run
[x] supporting services moved to hostname from localhost
[x] ensure one source of truth for php installation on devcontainer and GitHub Actions
[ ] couch db integration tests
[ ] memcached integration tests
[ ] get docker build --cache-from working in Actions
[ ] get xdebug working the devcontainer
[ ] push to packagist
