The top level heading appears in the table of contents and appears twice on every page. This is my own mistake: I duplicated the title of the document in Pandoc's YAML front-matter in the top-level heading for each section, e.g.,:

title: This is the document's title
---
This is the document's title

This happened when I was splitting the topics into different files, then just kept happening because I was copy/pasting folders.
I'll work on this piecemeal, and then try to remember to close this issue when I'm done. It will be painful because I will need to bring up all headers by one level.
