Fix: UMA team suggested changes #76

Spec discussion:

remove whenNotPuased and isUMAFailed check, to not block bond payouts
remove revert conditon in escalationManager
add check for set oracle price, to set only 0 or 1e18 price
add settle Aseertion function to settle policy id manually if uma failed
remove unsused state variables
add policy id, caller and payout address in insurance event
