Address a number of issues

• remove the old importer for now.
• remove koji from fletchling example config for now. The support for importing to it is gone until I re-add it. It will continue to work if anyone happened to configure it so far, but I think no one is using it.
• Fix the new osm importer to take a config file. I think this is a very simple config file.. (or I hope!) The uri methods on the command line was cute, but annoying because it required escaping certain password characters.
• Fix the README to address the above
• Fix nests with no names not being imported when they could be (Issue #6)
• Do not load spawnpoints on startup or reload unless they are not already known in the DB. An api call will be added to refresh spawnpoint counts in the future.
• Remove the spawnpoint caching. The query to get spawnpoints is now a COUNT instead of SELECT when we do need to get the spawnpoint count.
• Fix issue when golbat db is not configured. The spawnpoint filter was bypassed even when we knew spawnpoints from the DB.
• Add NestId to most logs with the nest name. This helps identify those with names like 'Unknown'.

I got confused as I didn't notice len's PR was against my branch... so in messing up a bit, I ended up merging this OOB. I think it's fine. :) (I did address the comments)
