Request that all users provide their departmental affiliation on registration #21

Departmental affiliation should be collected from users upon registration.

Users should be able to select one of the following options from a drop down list:

Arthritis Research UK Centre for Epidemiology (Manchester)
Arthritis Research UK Centre for Epidemiology (UEA)
Musculoskeletal Epidemiology Unit (Oxford)
MRC Lifecourse Epidemiology Unit (Southampton)
Academic Rheumatology Group (Bristol)
Epidemiology (Aberdeen)
Primary Care & Health Sciences (Keele)
Division of Rheumatology, Orthopaedics and Dermatology (Nottingham)
Other

If the user selects "Other" the user should be able to provide their departmental affiliation via a free text field.

The current implementation uses a free-text field - a drop down as described above with additional free-text field is required.
