Short description of the issue and how to reproduce (is it random / always happens / side conditions ?)

The problem can be easily reproduced. It is enough to take fall damage, and when you move you will receive damage until you die. I also noticed that when you die, chunks are unloaded. https://youtu.be/-CHI-p7sgZ4

I was only able to take a swift look at Spigot's commits, to see if anything relevant was changed, but no luck. I'll have to do some first-hand testings in the coming days.
