Switch placement of Birthday and Citizen verification #267

switch places with Birthdate and US Resident so that Birthdate comes first, then US Resident and then the check box at the end. Let's also make the outline of the checkbox was darker.

@adolfo32, we're still waiting on feedback from the client but I have a feeling this was an issue singular to the web app.
