Improved translation of mouse cursor position to joystick axis values. #110

Updated the translation of mouse cursor position to joystick axis values to take into account the border (including the border when force aspect ratio is enabled) around the active area of the display except when in full screen mode. Also removed handling of WM_SETFOCUS as the behavior is inconsistent with the general user experience on Windows platforms.

Seems okay. Verified mouse range is improved using several hi and low rez programs.
