Clean up database to remove access requests from staff

Once testing of the study mgmt dashboard & auto-emails is complete, clean up the database to remove all requests from staff that were made for testing/QA purposes. In addition to VEuPathDB staff accounts, other accounts that were used for testing include:

*Note, there may be other accounts used by staff for testing. Please check in with @nkittur-uga
