Include manager emails in the "CC" field of auto-emails as appropriate

Study team members listed as "Managers" on the dashboard should be included in the "CC" field of the auto-email that gets sent out to requestors in response to an access request.

Currently, managers are NOT included on the email, so there is no way for the requestor to communicate with the team.

Please make sure that manager emails are included in the "CC" field of auto-emails sent to requestors after request

Based on managers for JILINDE costing survey in Kenya, Nupur (gmail) and Danica (hotmail) should have been cc'd

@aurreco-uga @nkittur-uga and @danicahelb discussed, we don't want managers getting inundated with emails. Decision that managers will NOT be cc'd on the initial email informing users that their request has been received

Change this ticket to double-check appropriate cc email addresses on emails sent in response to approval/denial, once those emails are getting sent out
