study access service: data requestor table updates are not getting updated #89

using clinepidb.org, and signed in with my staff acct (dhelb@sas.upenn.edu), I 1st approved then denied the access request from helbda@yahoo.com

The "Notes" column on the Data Requestor table updated each time I made an action

However, there was no log of these changes made to the Data requestor table updates table:

We need to ensure every action made to the Data Requestor table is logged, including WHO performed the action (approve, deny, add data requestor or remove data requestor)
