Can you please tell me what audio sample rate are you using?
I went through every possible audio sample rate already, checked and unchecked boxes but the problem still occurs. I should also add that the sound FX are not affected but only the voices and the issue persists until the first CODEC communication where the voices resume back to stereo from there on.
Are these 2 options selected?
In alternative, you can try to use IndirectSound?
Just extract the 2 files from the zip inside the main game folder. Overwrite. Then launch mgs2_sse.exe
(Un)checking these boxes didn't make a difference before and after I applied IndirectSound.
So I guess that even with IndirectSound, the issue is not fixed?
With the original GOG release you don't have this issue?

IndirectSound didn't fix the problem.

No issue until I apply the patch.
I guess the only way to pinpoint what is fixing/breaking it is to apply only my audio fix to the original GOG version.
If you want, you could delete all (uninstall + manual delete of the game folder) reinstall the game using GOG release, run the game and check that everything is ok.
Then manually apply ON THE GOG RELEASE THIS audio fix.
Then do a simple test: is the audio broken or is working good?

Applying the audio fix alone to the GOG version without patching it doesn't break the sound.
Ok, is like I tought: there's some kind of incompatibility with other .dll I'm using.
Can you please send me your mgs2.ini you have now (without V's fix) and AFTER applying the V's fix?
Ok, let's recap:
Uninstall the game and delete everything inside the folder (backup your savegame!)
Reinstall the game from GOG (This way, you have a clear GOG installation)
Configure the game from GOG launcher/original launcher. (Like you like, we could say). Launch the game and check that the audio is working good.
Send me the mgs2.ini file (you can post the file somewhere here, on pastebin or in any way you like) you can find inside the main game folder.
Download THIS audio fix. Extract it inside the GOG installation you just made. Overwrite.
Launch the game from mgs2_sse.exe (inside the GOG installation folder) and check if the audio is working good or if it has issues...
I know that is annoying, but is the only chance I have to solve this 😛

Patch applied --- Voices: Left channel / Sound FX: Stereo => BROKEN
I included 2 mgs2.ini files for before and after the game got patched.
I used to have an audio bug during the opening cutscene of the Plant mission where I can only hear the voices in my left ear. I thought this problem has always been present in the base game until I decided to do a fresh install and play it without the patches and the voices are now in stereo as they should be. That is until I apply the patches again, whether it is 1.7 or 1.8.
