1 month ago I just suffered from the same issue as well. When I alt+R to self cast the spell to myself, I heard the sound effect of Guardian Angel, but there is no buff on myself.
I later tested the spell in demo mode, found that when I cast GA with alt+R, the effect applied onto my cursor location instead.
My Match ID was 7892187455 (timestamp could be at around 29:50).

Description
When Omniknight double taps Guardian Angel to self-cast, the ability goes on cooldown without applying the effect. With Aghanims scepter, double-tap self-cast will apply on Omniknight only and not on nearby ally heroes. (Same in demo mode)
