[TF2] Action item exploit. Possible to force other players to use their action items

At the moment, it's possible to use an exploit to trigger other players' action items.

There's a SourceMod extension to block this behavior, but it really should be fixed in the game itself.

Technical Summary: The use_action_item command can be issued with an entity index not belonging to the player who issued the command.
