I can confirm this occurs even in the latest prerelease beta.
The radar issue is very easy to replicate.
1). Change map to any custom map (de_season, de_contra, or de_tuscan). 2). Change map to any stock map. 3). Change map back to the custom map (de_season, de_contra, or de_tuscan).
Note: The radar glitch doesn't occur on stock maps for some reason.

I was hoping the update on 5/20 would fix this issue, but it has not. "sv_pure 2" remains unusable on custom maps.
I'd really like for this to be fixed. Let me know whether further information is needed.
After the first time playing the map CS:S doesn't handle materials and models bundled in the .bsp correctly. I have no idea why it works the first time around.

I really hope valve fixes this problem... It's going to be a pain in the ass to make a whitelist for every single custom map.

Do you have any updated repro steps? I just tried doing the steps via "create server" and it didn't produce the bug. Do you have Steam? I'd like to chat since we have a chance to get this fixed.
I just checked and I was able to reproduce the bug every time. I suspect you fell victim to #216.

Here my conclusion what I have tested.
The server is using sv_pure 2. (Fully pure)

Even with the latest CS:S update there is still a missing texture issue for maps. Also, the radar still doesn't properly draw the map.
