I tried to reproduce it, but in my case nothing gets removed. Only the default Playback Device changes to the HMD (from the monitor). Well, I tried 1.10.32 and unless I did it wrong it didn't help. I also tried this version from 2018 but that was error prone and didn't quite work right.https://steamcommunity.com/app/250820/discussions/0/1741101364288717349/

@Raivr-dev I've formatted Windows and the problem persists. One thing I neglected to mention that may or may not be important. I have taken the headphones off the Index as I use my own LG headset for audio.
I watched your video. It seems SteamVR changes the 4K60 Pro Mk2 audio output to Index HMD. Maybe the Nvidia card cannot handle two separate audio outputs in the current driver? You could try an older Nvidia driver. Other than that and trying an older SteamVR version, I don't have any ideas. So, Valve would have to try to reproduce this. (I don't work there, btw.) Also, I'm not sure if my monitor is connected to the same graphics card as the HMD. (I can't check atm.)

I have tested with 3 different HDMI audio devices. An Elgato 4k60 Pro Mk.2, which is what I have been using for about 2 years, an Elgato HD60S and an Asus ROG monitor with speakers.
In all instances when I start Steam VR, whatever device is plugged into the HDMI slot is removed from Windows device manager. I made a video for Steam Support that illustrates the issue.

I have contacted Steam Support but that hasn't really gone anywhere. I believe this is a bug in the latest release of Steam VR as it started happening after Steam VR updated on a system that's been functional for roughly 2 years.
I've cleared steamvr.vrsettings. Updated to the latest beta branch. We've uninstalled any third party audio components like Voicemeter which I use to get audio from my game PC to my stream PC. Again, working well for 2ish years. Reinstalled GPU drivers. DDU uninstalled. Disconnected the wireless receiver on my Logitech headset and tried again. Verified everything is connected to my primary GPU. Tried a clean boot of Windows. Disabled SLI.
Steam Support then asked to remove the HDMI audio device and see if the problem persists.... I didn't do that one. If there's no HDMI audio device for Steam to kill, obviously the problem isn't going to happen...
On my own I've uninstalled and reinstalled both Steam VR and Steam as a whole. I've tried to find and delete any cached information that Steam might have left behind. Rebooting after each uninstall. I've tried rolling back drivers. I've tried using DDU to clean audio drivers. Replaced the HDMI cable. And a variety of other things I've seen online for similarish issues.
REPRO Steps: Get a monitor with speakers. Get a GPU with an HDMI out. 2080ti in this case. Plug the monitor with speakers into the HDMI port. Open Device Manager and find the audio device for the monitor. Start Steam VR. Observe the HDMI audio device being removed from Device Manager.
