improve contact us #48

Right now the “Contact Us” screen simply contains an email address. Could the title and contents of this screen be changed to be more inviting and facilitate comments? [maybe “Your feedback” – I’m guessing Brumfield Labs has a better idea] Perhaps there might be a box in which someone could enter text and just hit a button and have them emailed to me, and / or maybe some sort of optional rating scale. Maybe a question / response box such as “how might the Curran Index better meet your needs?” I would appreciate suggestions based on your past experience. It also looks like my email address is hard coded on this screen. If so, it shouldn’t be.

@gsimons1946 take a look at that and see what you think. I find it's easier to write when someone takes a first stab at it, and you can change, fix, and improve what they did.

I think you should also think about how people can improve the information in the CI. Maybe a different form (or just a separate paragraph in the above) about contributing more or updated data back in?
