Strix's thoughts on this:
I think users are confused by uploads for two
reasons: they don't know to open the uploads dialog, and they don't really
understand files and folders.
They probably want to put the image into the text, the attach and embed
process is less intuitive. Behind the scenes it must work this way, but a
file upload box and drop target could be placed directly in the page, and
bound to the textarea, something like gmail does.
One can detect mousemove over the textarea and if the mouse is carrying
files from a drag, place a drop target directly over the WYSWYG editor as a
translucent modal, and show the uploads there, with a quick instruction
about using the 'attach media' button on upload complete. Something the
user will have to click 'OK' to when the files are attached.

I have thought about this one. The VSA invented the awarenet programme to teach
learners basic computer skills.
Understanding files and folders is actually a very basic computer skill, so we
should make an effort to teach it to the learners as part of the programme.
