implement multi-school functionality for Kalite Implementation

The Kalite implementation at the moment does not cater for a many school 
awarenet installation. Result of this is that any teacher or admin is able to 
check progress of any student, even if the student is at a different school.

Goal of this task is to link each school in awarenet to its own Kalite 
facility. With this each teacher and student can only operate within their own 
facility.

Conceptually this is easily done. Functionality wise I need to implement new 
awarenet code that can automatically create a Kalite facility for a school 
(ideally called at the creation of the first user of a school) and then start 
making use of the facility parameter for the coach reports and video/subtitle 
downloads as well as the watch video and do exercise functionality.

I will link this activity with issue 383 so that with this change the Kalite 
Integration moves into its own package.

Closing this, we can use a single KA lite school / institution for any number of awarenet schools - from KA side it's all the same.
