Create accounts db model according to this image. We can expand upon them later.

We'll need:

Caregiver

contains caregiver's info
perhaps a one-to-many model of assigned U18s (please confirm this with Thomas via Slack)

Youth - (U18)

This will be just an id to be provided by a caregiver
This model is used to store tracking data for document progress
