Update the spec with upstream changes in HTML spec (revealed by Bikeshed errors).
In particular:
"session history" is no longer a concept on browsing context. This patch removes the entire step referencing it in 'create and initialize a Document object' since it's checking for a same-document navigation. This doesn't make sense in the create Document steps.
"indicated part of the document" is now a Document's "indicated part". Also cleaned up the relevant sections as they were updated and made it clearer what's being replaced/removed/kept.
"child browsing contexts" is also no longer a concept. The bit of prose referencing it is removed since the session history entry doesn't restore scrollable regions that are navigable containers (i.e. what we previously would have called a child browsing context).
