Create a new issue from johannhof feedback in #218. I want to merge #218 since it has other fixes that would improve the spec.
Line 734: I'm not an expert on structured headers but instead of "pairs" I think you need to construct an inner list here? Line 735: I might be wrong but that seems unnecessary to me. Line 737: Please use set a structured field value as in https://wicg.github.io/trust-token-api/#append-private-state-token-issue-request-headers.
