Significant rewrite of the explainer #2

1) Provided more background on how apps are launched currently and in the future 2) Restructured motivating examples 3) Altered definition of what happens if event.preventDefault isn't called. In this case the default browser behavior should happen. 4) Added a section on design questions and filled it in with questions that have come up 5) Restructured the document
