I saved a T3 game; a 3 AI enemy team me against me. The 3 AIs all have a Laser Satellite Tower up and firing away. The AIs are also pretty predictable in that they go after my HQ and one of factories pretty religiously.
To counter this, I have ensured that several of my trucks are parked nearby my row of factories, just out of the splash damage range from the LasSat strike radius. Some time before I saved the game, the AIs took out my factory and HQ, and I issued my trucks orders to rebuild them both. Of course, I have to come back later and issue my trucks orders to upgrade my factory with a T2 module, and finally a T3 module, which I do. Since my trucks were following queued build orders and the factory module was the last thing in the queue, 5 minutes go by and the AI enemies have their LasSats ready to blast away at my new Factory again before the Factory module construction was started. So the AIs reduce my shiny new factory to embers (which sits on the same tiles as the factory they targeted earlier).
But this time, the green Factory module hologram that marked my build orders remains behind, without the factory under it. It will still show up after you load the saved game too.
Trucks can still build a factory on the same tiles, and I believe the green hologram will go away after the factory module is eventually built.
It's a repeatable issue as well, not a one time thing. I saw it several times.
While I haven't seen it, I would suspect the same thing would also happen with research facilities and VTOL factories.
