customize id page to recruitment platform #817

At least one recruitment platform manager doesn't like that our ID page lists other recruitment platform IDs (ie, "enter you platform assigned id, eg. mechanical turk id, prolific id, etc") as they believe this confuses participants.

We should be able to customize our participant ID page to the platform we recruit from so that we ask specifically for the right one. We could do this using the "referrer" property on the page for MTurk and prolific, or by setting a config option.

One way to do this would be to be able to specify a markdown file similar to the consent addendum that is used as the instructions, with a generic fallback if none is specified.
