Restructure as NPM package #66

Restructures surveys repo as an npm package that can be installed into a project and surveys can be loaded from.

This way, we can include in node projects with npm -i @wattslab/surveys

Includes cypress tests of each survey that also check that the survey aggregation function is working properly
