Hey, the matrix question is formatting super strangely on my computer - the prompts are all squished up and hard to read, and then the radio buttons are so spread out that only half of them fit on the page.

I started playing with using the "rating" option instead of the matrix question, which looks easier to use:

how would you feel about switching to this instead? I can update the survey.json with this example...

Downside to this approach is that you don't get to label the middle value as "moderately". Do you think that's ok?

@sky1113 I went ahead and restructured away from the question matrix, I hope that's alright with you. =) Lets copy this style for the self-listening quesionaire
