When running this on a server with redstone in the current configuration:

The blocks should be swapping locations, however over time they will eventualy sync up and be pushed backwards and forwards together rather than alternating.

This is consistant with other piston block activity in other areas especialy when powered by different forms of restone.

Interesting issue. It appears to be a problem with the redstone/loading tho. Have you made sure the behaviour changes reproduceabley from a clean vanilla installation vs a dimthread only(!) installation?

I have this issue too. It only occurs in the nether on my server. The bug is 'breaking' bi-directional slime flying machines causing them to flip travel direction early. This is together with other mods though. I haven't fully tested if it also occurs only with dimthreads.
