Types and instructions reference #96

I've been prototyping out an implementation of interface-types and collected some notes on types/instructions. I'm posting this here to see if this matches the working model other people have been using.

I've updated the document for review comments and dropped the more controversial design points that are expected to take a longer time to resolve.

I've updated the document again for the changes agreed upon in our last meeting. I think I got everything, but I didn't write down good enough notes then, so I may have forgotten something.
