Restructure specification to match new approach #13

The current specification is out of date and does not contain the key approach landed on recently. Specifically, the spec needs to define:

[ ] The relationship to did:web
[ ] Support for exposing DIDs also as did:web DIDs
[ ] How to structure DIDs
[ ] Sections for how to generate the DID Doc from KEL, rpy, etc.
[ ] The contents of the DID Resolution Metadata
[ ] CRUD method definitions.
