Refactor to make managing upstream changes from coinbin and btcr-did-… #31

This is to resolve issue #30

I'm pretty sure that the only modifications need to be made to index.html so we can keep the coinbin and btcr-did-util-js repositories separate. I've added instructions to import the repos as git submodules which should simplify things somewhat.

The changes to the coinbin .is files were to add support for BTC testnet. A pull request was made to the coinbin repository for this but the API endpoint has since been retired. Support for testnet has been included now though. See https://github.com/OutCast3k/coinbin/pull/188 for details
