feat(definitions) identity & identifier #34

This PR takes a stab at actually defining what identity is, instead of being all hand-wavy about it. It also follows this definition to better describe identifiers.

Not a bad improvement, accepted. We really should update it with the definition from the primer.

@ChristopherA - Don't know if you are addressing this at me as I am not the Author of the original set of definitions and I don't think I will be able to attend RWOT9, unfortunately. I am just glad to participate for now.
