Add stub for usage scenarios & challenges document #1

@beverloo @qingan I went with ReSpec as our authoring tool; it currently raises an error due to https://github.com/w3c/respec-web-services/issues/279 which hopefully will be fixed soon

If you can give a quick sanity-check that this is reasonable stub, that'll be appreciated
