rapid assessment integration

Feature description and context

giraffespotter has the rapid assessment feature. To finish integrating this custom work into the main code base, we need to address the following:

Currently in the giraffe branch, encounters have the "complete" state, set via hacky use of SystemValue (done through the "mark complete" button in the RA page). We should

• add a new encounter.state quick reviewed
• read that state in for the rapid.jsp page when showing if an encounter is reviewed or not
• have that be set on the encounter when "mark complete" is clicked

Feature sign-off test pattern

1. bulk import leads to rapid assessment (current function in GiraffeSpotter)
2. user clicks Mark Complete
3. encounter state are set to quick reviewed
