Create warnings related to Flask results limit #17

The limit in $flask_url was causing us to miss annotations in the TEI annotations file.

Add warnings to converter output when approaching and hitting the limit of results. Also add note in config file that if raising the limit, it must also be done in annotonia-status and annotator-store
