When using the Artifact of Escalation and visiting Sky Meadow, no normal enemies will spawn at all: only Boss spawns are allowed, and only after a certain amount of time has passed OR if one or more mountain shrines have been triggered before starting the Teleporter event. This issue seems to be related to how the director credits system works. Combat shrines will still normally spawn enemies. So far I've been able to spawn either Grandparents or Scavengers in my few attempts. This issue doesn't occur in any other stage, from what I've seen.

I've done a couple runs and have not been able to reproduce the issue. If it keeps happening can you share a log so I can see if anything weird is happening.

I apologize for assuming this was an issue intrinsic to the artifact. I've done a bit of testing and it seems to be a weird interaction between a very specific setting in Moffein's BossConfig mod and the artifact itself. Specifically, disabling Scavenger spawns in any stage will completely bork enemy spawns in that specific stage, leading to no normal enemies spawning but only Bosses, including, strangely enough, the Scavengers themselves (even though they've been set to false). This must be related to the fact that Scavengers are normally a loop-only boss, hence conflicting with the artifact of escalation.

Huh I'm not really sure how disabling scavs would bug out the stage. The only change I did for monsters to appear earlier is to change it so that the stages cleared requirement is 0.
