Make header a test/debug feature #25

Currently, we display a form at the top of the page to select the org and repo.

This feature, as of now, is only useful for debugging purpose since it's not supposed to be displayed on GitHub and we haven't yet done any work on thinking how a standalone board would work.

We should have some sort of debug flag/variable that allows us to load it (either using configuration or using a setting in the URL). By default we should hide it IMHO.

Done, we are using ?toolbar=true for displaying the header.
