Implement app icons where necessary #112

Simply replace all the default Java coffee icons with the appropriately-sized 
VisiGraph application icon, in preparation for the creation of 
platform-specific launchers/installers (see Issue #110 and Issue #111).

Per r138's check-in notes:
"In the process, I further optimized the 16x16 app icon for better subpixel 
rendering.  Unfortunately, I also ran into what appears to be an outstanding 
bug in the Java Swing framework, which prevents me from changing the frame icon 
of an InternalMessageDialog from the default Java coffee cup icon (Note that 
this does not apply on Macs, as they do not utilize *Internal*MessageDialogs, 
due to another existing bug in the Swing framework). Additionally, a 12x12 icon 
was added for use in JInternalFrames."
