To create Form for Party poppers

create a form for the users who will use our app as Party animals or who want to use event organizers service.

Fields which you will be adding in the form-

1. Phone num (which you will already have through login with the option of editing) - editing functionality will have another ticket.
2. Name
3. birthdate
4. Gender
5. city
6. Interest ( dropdown with multiple options selection)
7. with Proceed button
