Create Form for Event Organizers

create a form for the users who will use our app as Event organizers or who want to use give the service of organizing various events.

Fields which you will be adding in the form-

1. Phone num (which you will already have through login with the option of editing) - editing functionality will have another ticket.
2. Name
3. birthdate
4. Gender
5. city
6. organising( dropdown with multiple options selection)
7. with Proceed button
