Add KPI Definition (Gatekeeper)

As a gatekeeper, I want to have an option to Add New KPI to be able to define KPIs for the circles.

Acceptance criteria:

• [x] KPI definition page has an option Add New KPI.
• [x] Upon clicking on Add New KPI, a form opens.
