Add a Bug report template #21

I think this template is too complicated to this project.

This template is completely optional.

Of course, We could just place a gif or png image if that is enough.

The issue template could help to open an first-issue and describe issue as developer-friendly.

In my experience, usually someone who wants to report an issue could feels discouraged by blank. Because they didn't know how to describe it.

How about adding some line this is an optional template or Please describe a bug freely. If you need issue template, please see below ?

BTW, I don't think this is too complicated. Because I think reproduce steps, expected(correct) behavior and actual(problem) behavior is minimal information for 'bug report'.
