Another batch of improvements

Improvements included in this batch of changes:

• Fix names like ".pdf (hq)" to be ".hq.pdf"
• trim filenames (some have an extra space at the end)
• reorganize name cleanup to make space removal easily optional
• Keep track of errors and report them at the end, but keep trying other files
• Move extension mangling to a subroutine
• Move MD5 checking to a subroutine and also check filesize (much faster)
• For filename cleaning, simplify the check for sequences to replace with a dash
• Handle "read me" files that don't have a download URL and need to be skipped
• Improve listing output with some small tweaks

This is everything I have in mind for changes I am ready to make right now. With these changes, the script perfectly suits my needs.

Note that I tested each error condition. I tested several of them by deleting one file and changing to code to force the error case to trigger for every download. Kinda brute force, but it worked.
