I've gone through all of the code that is rated C or less. I've made a few changes that should improve things. Some of the other stuff isn't really bad enough to warrant a fix. Some of it probably should see some work, but will be tackled elsewhere as the plugin evolves. And some of it will be removed one day in a future version.

Some of the areas with duplication were cleaned up in work on #307.

Going to close this. If we find more issues we want to fix in 2.0.0, we can reopen.
