Add `CONTRIBUTING.md` #79

Split out the initial contribution information from readme.txt, so we can include more detailed information and code of conduct.

This could also be a good place to document our other policies/practices/flows with the project.

Maybe something like Gutenberg's?

Just a quick note that this should also include coding standards, even though this is enforced by the tests/wpcs.
