First pass at six intl examples (patients, immunizations, organizations) #43

This creates example Patient, Organization, and Immunization resources for English, French, Spanish, Chinese, Russian, and Spanish. Please note that localization in FHIR is problematic. For example, FHIR IDs must be ASCII.
