Health Prevention is missing for Older child #27

The health prevention questions are appearing in Assessment questionnaire, which is not right

Yes, The form on health prevention was not showing since the plan definition is not working yet. I have implemented it in the Assessment test as a workaround since we can import questionnaires there as a workaround for now, is it okay for you Dipti? @delcroip

Yes it's related to this comment from #13 , since the plan definition is not yet working (responsible for gluing the questionnaires together) there are only two options, do it like the other questionnaires on L4 level and add it to the sequence OR we add it to the assessments as a work around (since in our input file it's still a separate questionnaire and we don't have to manually take it out of the assessments, we just remove the "import" row)
