Help info to be updated as per DAK #151

Help info is now integrated in DAK . Please refer to latest DAK and use the text from column "Description/User information"

I started to review help info and below are few observations 1.Help info is missing axillary temp question in Measurements 2.Help info does not match the file text for "Convulsions in this illness" 3.Help info is missing for "Weight cannot be measured" question 4.For check box questions "Thermometer not measured" help is appearing above the element rather than below. @ppanchal-argus your intervention is required for this point.

[x] 2. was inverted with "Continue to Assess Sick Child" with
[x] 3. same as 1

I do my testing on 1.0.3.build.52
