'Al-Seleikh 2' and 'Al-Seliekh Al-Thani PHCC' are the same health facility

'Al-Seliekh Al-Thani PHCC' is the one we need to keep

need to please:

check is any users have been assigned 'Al-Seleikh 2' and change assignment to 'Al-Seliekh Al-Thani PHCC'
check if any data submitted against 'Al-Seleikh 2' and if so move it under 'Al-Seliekh Al-Thani PHCC'
deactivate 'Al-Seleikh 2' so cant be used
