Fever for how long showing inappropriately #60

High temperature (from axillary temp >=38.0), with 'no' response to 'fever reported' should result in 'fever for how long' being auto-answered (as per DAK) and not shown.

Cannot do that for Now because of the lack of support of the %terminology
